Home Crafter Royalty Report

Based on terms outlined in your Crafter License Agreement with the University of Utah, you must submit a royalty report each quarter, due 30 days after the end of each quarter.

This report must include your total sales figures, the total number of licensed articles sold, the number of holograms used, the number of hologram labels unused, and specific locations where product sales were made. Please submit a form even if the sales/number of items sold was zero.
